Medical documents required
at the beginning of the school year
The Avenor Medical team is prepared to treat the health of our students with the utmost care and responsibility. However, we also rely on your support and cooperation to ensure that together we can keep the community safe.
New students, who have not been part of the Avenor community in the previous school year, are required to provide medical documents until August 23rd.
THESE DOCUMENTS NEED TO BE UPLODED TO THE PERSONAL ACCOUNT ON OPEN APPLY. We cannot accept documents in physical form.

List of compulsory medical documents for new students in 2022-2023 school year:
Medical certificate for enrollment in the community (issued by the family doctor)
Adeverință eliberată de medicul de familie cu specificația „apt pentru susținerea orelor de educație fizică și sport”(se aduce în prima săptămână de școală.)
DOAR PENTRU ELEVII NOI:

List of compulsory medical documents for NEW STUDENTS in 2024-2025 school year:
Medical certificate for enrollment in the community (issued by the family doctor)
Vaccination form (issued by the family doctor)
Epidemiology certificate (issued by the family doctor) – with an availability of 48 h
The child’s medical history (filled in by the parents)
According to Order no. 1002/5057/2015 issued by the Ministry of Health, the certificates are valid only if they are filled in on the standard forms that you can find in the links above.
